Social Services: Supporting Residents and Families Through Every Step

Making the decision to move into a new home is often a difficult one—for both residents and their families. Frequently, placement follows an acute medical event requiring hospitalization. Whether the stay at Oakview is short-term or long-term, our staff is highly sensitive to each individual’s needs and the emotional impact of such a transition.

During the adjustment period, residents may experience feelings of loss, uncertainty, or anxiety. These emotions are gently and respectfully addressed by our trained Social Services team, who also offer support to family members navigating the change. Additional psychosocial needs are assessed and addressed on an individual basis to ensure personalized, compassionate care.

Oakview’s social workers are professionally trained, holding degrees in Social Work, and play a vital, evolving role within the facility. With the growing focus on Quality of Life in long-term care, our social workers advocate for the dignity, autonomy and well-being of each resident. Their responsibilities include:

  • Participating in Behavioral Management Team and High-Risk Falls Committee initiatives

  • Monitoring and helping minimize the use of psychoactive or mood-altering medications

  • Promoting a restraint-free environment by understanding and addressing behavioral causes

  • Collaborating closely with the interdisciplinary team and attending physicians

  • Exploring non-pharmaceutical interventions to improve mood, behavior, and overall wellness

A core principle of social work is recognizing how environmental factors affect daily living. Our social workers are proactive in creating supportive surroundings that reduce stress and enhance quality of life.

Additionally, the Social Service staff maintains strong relationships with Corewell Health Hospital, Community Mental Health, Home Health Care agencies, Probate Court and other community organizations to ensure residents receive comprehensive and coordinated care.

As liaisons between the resident, their family, and the facility, social workers assist with important decisions such as advance directives and help navigate the complexities of long-term care planning. Above all, they are dedicated to ensuring the medically related emotional and social needs of each resident are met and upheld throughout their stay at Oakview.
