These evidence-based interventions are tailored to each resident’s specific medical needs and rehabilitation goals.
A physician’s referral and diagnosis are required to initiate skilled Physical Therapy. Once deemed medically necessary, an individualized treatment plan is developed. Therapy is then coordinated with the entire interdisciplinary care team, including the resident and their family, to ensure a collaborative and person-centered approach.
Our goal is to help each resident achieve the highest possible level of function and independence, with many returning home to their families and communities upon completing their rehabilitation program.
Oakview is equipped to meet subacute and complex care needs through an integrated team offering, In-house Physical, Occupational, Speech, and Recreational Therapies, 24-hour Skilled Nursing Care, On-site visits from attending physicians and a specialized long-term care pharmacy. This holistic approach ensures that each resident receives the care and support needed to recover from surgery or injury—or, when appropriate, to live out their final stage of life with wellness, dignity, and compassion.
Therapy services are available Monday through Friday, with weekend sessions provided as clinically indicated.
